By submitting this form, I grant permission to Tattoo Warriors, operated by Kevin Reyes, to photograph, film, and record me during the event.

I understand that this content may be used for promotional, marketing, advertising, and commercial purposes, including but not limited to social media, websites, digital advertising, and future promotional materials.

I acknowledge that I will not receive compensation for the use of my image or likeness and waive any right to inspect or approve the final content.

I release and hold harmless Tattoo Warriors, Kevin Reyes, and all affiliated parties from any claims, demands, or liabilities arising from the use of my image, likeness, or voice.

I confirm that I am 18 years of age or older, or I have obtained consent from a legal guardian.
